Please read before starting surveys

Below are some key items that guide which survey form you should be using. Please review the descriptions of each type of program so you download and fill out the appropriate form for your program.

Below each description is a copy of a fillable PDF form that you can use to collect the data. The online survey requires you enter all the data at one time. You cannot start and return to the survey. We encourage you to fill out the PDF form and use it to enter your data. (As always, if time becomes a barrier to you entering the data, you can download the form and collect the data on that form. Then email the completed form to info@aabh.org.

PHP Only Survey

Use this survey if you program is a stand alone PHP or distinctly separate from your other programs.

IOP Only Survey

Use this survey if you program is a stand alone IOP or distinctly separate from your other programs.

Combined Program Survey

Use this survey if you program offers both a PHP and IOP together, but you track them separately. You use the same staff and space, but track visits separately.

Integrated Program Survey

Use this survey if you program is a single program that has PHP and IOP included, but your data is combined as a single program. Often the patient may not even know if they are in PHP or IOP. Patient groups are combined and there is no separation of programs. Only billing and documentation recognize the different levels of care.
